By default they see what is assigned to them. Broader visibility is a permission you grant deliberately.

Does assignment notify the agent?

Yes, and the account owner. The matching CRM lead is assigned at the same time.

What stops two people replying at once?

Assignment, used before replying rather than after. That habit is what prevents the most common shared-inbox failure.
